Responsibilities
- Perform PT evaluations and develop individualized treatment plans for patients in a home health setting.
- Implement and adjust therapy interventions, including therapeutic exercises, gait training, balance, ROM, and neuromuscular reeducation as appropriate.
- Monitor patient progress, document outcomes, and update care plans in compliance with regulations and payer requirements.
- Collaborate with the supervising PT, other clinicians, and family/caregivers to coordinate care and ensure a safe home environment.
- Educate patients and families on exercises, safety precautions, and home program adherence.
- Maintain accurate documentation and uphold HIPAA and professional standards in all records and communications.

Requirements
- Active Physical Therapist license in the United States (or eligibility to practice).
- Doctor of Physical Therapy (DPT) degree or equivalent.
- Ability to travel to patient homes within the Gainesville, VA area to deliver in-home PT services.
- Strong documentation, organizational, and communication skills.
- Home health experience preferred; candidates without prior home health experience but with relevant clinical PT experience will be considered.
