Primary Responsibilities:

• ISP Collaboration: Assistin the creation and ongoing revision of ISPs tailored to the needs ofeach individual.

• Ensure Plan Implementation Monitor and ensure that ISPs are being effectively carried out.

• Case Management Provide linkage to resources and case management support for individuals.

• Timely Documentation Submit necessary documentation according to establishedtimeframes.

• Collaborative Approach Maintain continuous communication with individuals and key stakeholders (with signed authorization) to ensure coordinated planning and service delivery.

• Additional Duties Complete other responsibilities as assigned to support the overall success of the program.
  

Qualifications:

• Educational Requirement: Bachelors degree or higher in a Human Service field

• Licensing: Valid registrationwith the Virginia Board of Counselingas a Qualified Mental Health Professional (QMHP), Qualified Mental Health Professional-Trainee (QMHP-T), or QPPMH(with certificate).

• Background Check: Successful completion of criminal background and central registry checks.

• Driving Requirements: Valid Drivers License, current auto insurance, and reliabletransportation.

• Healthand Safety CertificationRequirements:
  Current TB screening and CPR/First Aid certification(BLS not accepted)or willing to obtain
  priorto start date.

• Skill Building & Caregiver Support: Ability to provide various skill-building activities, including community and daily living skills, while enhancing caregiver roles andassistingindividuals in developing positive relationships and coping strategies.

• Behavior, Symptom & Crisis Management: Ability toassistindividuals in developing behavior and anger management techniques,monitorand manage mental health symptoms, and effectively handle crisis situations,maintainingcomposure and professionalism even with hostile or volatile individuals.

• Confidentiality:Ability tomaintainconfidentiality andcomply withHuman Rights and HIPAA regulations.

• Documentation:Ability to develop and write detailed progress notes and reports, documenting direct or collateral contacts.

• Aftercare Planning: Ability to develop and implement closure and aftercare plans for individuals and families.
