Overview

Responsibilities
- Perform clinical assessments to identify patient needs and appropriateness of physical therapy intervention.
- Develop, document, review, and revise the Plan of Care (POC), progress notes, and related clinical documentation; coordinate plans with the physician and care team.
- Plan and deliver treatment for neuromuscular, neurological, and orthopedic conditions based on medical referrals and assessments.
- Instruct patients and families on transfer techniques and safe use of equipment (e.g., walkers, wheelchairs, canes, braces, prosthetic devices); select and adapt rehabilitative equipment as needed.
- Provide patient/family education tailored to age, condition, culture, and learning readiness; promote home exercise programs and exercise safety.
- Collaborate with the interdisciplinary team; supervise and guide other staff as appropriate within scope of practice.
- Apply infection control principles and universal precautions in all patient care activities.
- Accept clinical assignments consistent with education and competence to care for patients; participate in ongoing professional development.

Requirements
- Active Physical Therapist license in the state of Virginia.
- Current CPR certification (required).
- Current driver’s license with reliable transportation and insurance.
- 1–2 years of PT experience in an acute care or rehabilitation setting (required); prior home care experience preferred.
- Education: Bachelor of Science in Physical Therapy from an American Physical Therapy Association–approved program (required).
- Ability to work in an acute care/rehabilitation setting and to travel as needed.
