- Core responsibilities include providing direct, hands-on nursing care to long-term residents under the supervision of a registered nurse and physician; administering prescribed medications and treatments; monitoring vital signs; documenting progress; contributing to care plans; assisting with activities of daily living, mobility, hygiene, wound care, and fall prevention; coordinating with physicians, therapists, and social services to support each resident’s goals; and identifying changes in condition to communicate needs promptly.
