Outreach Case Manager

Position Summary:
FACETS is seeking a passionate and mission-driven Outreach Case Manager to join our Region 2 Outreach Program, based out of Bailey’s Community Shelter. This dynamic role brings critical, person-centered services directly to individuals living unsheltered in woods, campsites, parks, abandoned buildings, bus stops, and other outdoor environments. Outreach staff actively connect with people experiencing homelessness to build trust, reduce barriers, and link them to essential services.
The Outreach Case Manager plays a key role in helping clients move from the streets into stable, permanent housing, often within 30 days. This is an exciting opportunity to make a direct, meaningful impact on the lives of some of Fairfax County’s most vulnerable residents.

Duties and Responsibilities:
The Outreach Case Manager will provide services or activities necessary to assist program participants in locating, obtaining, and retaining suitable permanent housing, including the following:
- Establish contact and build rapport with single adults experiencing unsheltered homelessness in Region 2 building relationships to provide immediate and long-term support.
- Provide clothing, toiletries, non-perishable food, and other supplies to clients.
- Refer clients to community-based and County-provided support services and resources to provide for their housing, employment, benefits, and other needs
- Collaborates with the Fairfax County Health Department’s Homeless Healthcare Program and the Community Services Board’s PATH Team to coordinate access to physical and behavioral health services for individuals experiencing homelessness.
- Assesses housing barriers faced by individuals experiencing homelessness to identify the client’s housing and support service needs.
- Develops individualized case management plans focused on housing acquisition, budgeting, and self-sufficiency, beginning with intake interviews to assess household needs, goals, and eligibility.
- Utilizes the Housing First model to deliver housing stabilization services and divert households from entering emergency shelters.
- Establishes and maintains consistent verbal and written communication channels, among operations staff, referral sources, and collaborating agencies to ensure effective coordination and information sharing.
- Advocates for and actively assists individuals in accessing a range of services, including mental health, intellectual disability support, substance use treatment, housing referrals, financial assistance, home based services, training, medical care, mentoring and socialization.
- Work an adjusted work schedule, including evenings and weekends as necessary to meet program needs.
- Performs other duties as assigned.
Required Skills/Abilities:
- Bachelor’s degree in applicable human services field plus two years of professional social work/case management experience with homeless and/or at-risk populations.
- Must be comfortable going into woods and other places not meant for human habitation to meet program participants.
- Ability to communicate clearly and concisely, both orally and in writing.
- Proficient with Microsoft Office Suite or related software, including Microsoft Excel.
- Candidates must possess a valid driver’s license, reliable transportation, good driving record, and personal car insurance. Candidate must be able to drive 15 passenger vans.
- Candidates will be required to pass criminal background checks.
- Bilingual (English/Spanish) is preferred.
Physical Requirements:
- Prolonged periods of sitting at a desk and working on a computer.
- Must be able to lift 15 pounds at times.
- Must be able to attend occasional meetings at other FACETS office locations within Fairfax County, including 3 annual organization-wide fundraising events each year.
Salary Range:
$47,000 - $51,000 annually.
