We are seeking a PRN Physical Therapist to join our dedicated team. This role offers flexibility and the opportunity to make a meaningful impact in the lives of patients by providing exceptional in-home physical therapy services. Key Responsibilities:
- Conduct comprehensive assessments of patients' physical condition and functional abilities.
- Develop personalized treatment plans tailored to individual patients' needs and goals.
- Implement therapeutic exercises, techniques, and modalities to improve patients' strength, mobility, and range of motion.
- Educate patients and their caregivers on proper techniques for exercises and activities to promote independence and prevent re-injury.
- Monitor patients' progress and adjust treatment plans as necessary to achieve optimal outcomes.
- Collaborate with other members of the healthcare team, including physicians, nurses, and occupational therapists, to ensure coordinated care and maximize patient outcomes.
- Maintain accurate and timely documentation of patient evaluations, progress notes, and treatment plans in accordance with regulatory requirements and company policies.
Qualifications:
- Bachelor's, Master's, or Doctorate degree in Physical Therapy from an accredited institution.
- Minimum of 2 years of experience as a Physical Therapist
- Current licensure as a Physical Therapist in the state of Virginia.
- CPR certification.
- Strong clinical skills and knowledge of evidence-based physical therapy practices.
- Excellent communication and interpersonal skills, with the ability to effectively collaborate with patients, families, and healthcare team members.
- Compassionate and patient-centered approach to care.
- Ability to work independently and manage a diverse caseload effectively.
- Prior experience in home health care or a similar setting is preferred but not required.
- Experience with Home Care Home Base (HCHB) a plus
