About the Role
As a provider, you will conduct thorough health assessments that can last up to one hour, providing essential insights into a member's medical history, current health status, medication adherence, and social environment. The Matrix Comprehensive Health Assessment (CHA) not only improves the quality of care but also helps in addressing multiple care gaps in one visit. Your responsibilities will include:
- Performing adult and geriatric health assessments, including medical history evaluation, diagnosis and treatment, health education, and necessary referrals.
- Administering and ordering point-of-care and lab-based tests as required.
- Collaborating with physicians, case managers, social workers, family members, and caregivers.
- Working closely with Primary Care Physicians (PCPs) on patient education and follow-up care.
- Providing services at the member’s residence, which may include their homes or Skilled Nursing Facilities.
Benefits of Working at Matrix
- Consistency: Full-time providers typically work Monday - Friday from 8:00 am to 5:30 pm, even though assessments are conducted Sunday - Saturday from 8 am - 7 pm.
- Competitive Pay: Enjoy an annual base salary ranging from $105,500 - $168,400 depending on your experience and market compensation analysis.
- Incentive Program: Eligible for a monthly (uncapped) incentive through the FT Clinician Traveler Incentive Plan. In 2025, average incentive payouts were $500/month ($6,000 annually).

- Licensure Support: We assist you in obtaining additional state licensure and credentialing as needed to expand your service areas.
Qualifications
- Master's Degree or equivalent experience with valid NP or PA licensure.
- Current NP/PA licensure in your state, including prescription authority, or willingness to obtain it as necessary.
- Board certification (NCCPA, AANP, AACN, or ANCC) in appropriate specialties.
- Current BLS, ACLS, or CPR certification.
- At least 1 year of NP or PA experience.
- Able to travel up to 80-90% of the time, including overnight and weekend travel across the assigned state and possibly neighboring states.
- Valid driver's license and appropriate automobile insurance coverage for your state of residence.
- Strong computer skills and familiarity with health record software.
- Excellent verbal and written communication skills.
