Role Overview

As a healthcare provider with Matrix, you will conduct thorough assessments lasting up to one hour, focusing on a member's health, medical history, medication adherence, and social risks to enhance their overall well-being. Through our Comprehensive Health Assessment (CHA), you'll play a crucial role in bridging care gaps in just one visit. Key responsibilities include:

• Performing assessments for adults and geriatrics, including medical history, diagnosis, treatment, health education, referrals, and clear documentation per Matrix protocols.
• Ordering and conducting point-of-care and lab-based testing as needed.
• Collaborating with physicians, case managers, social workers, families, and caregivers as appropriate.
• Coordinating with primary care physicians on patient education and follow-ups.
• Providing care at members' residences, be it their homes or Skilled Nursing Facilities.

• Competitive Pay: Earnings vary based on completed assessments, with an average of $1,800 to $11,500 monthly for those completing 20-100 visits. Compensation per assessment ranges from $90 to $115 depending on your home state.

Qualifications We Seek:

• Master's Degree or equivalent experience with satisfactory NP licensure completion.
• Active NP licensure in your practice state with prescriptive authority or the ability to obtain it.
• AANP, AACN, or ANCC board certification in one of the following specialties: Adult, Adult Gerontology Primary Care, Family, Emergency, or Acute Care.
• Current certifications in BLS, ACLS, or CPR.
• Preferred one year of NP experience; new graduates are welcome to apply!
• Valid driver’s license and auto insurance required in your state of residence.
• Proficiency in computer skills and familiarity with health/medical record software.
• Exceptional verbal and written communication abilities.
