Resource Specialist - Williamsburg
Reports to: VP of Care Coordination
FLSA status: Part-Time (24 hours/week 3 days - 8:30am - 5:00pm)
Non-Exempt
GENERAL PURPOSE
Under the supervision of the Care Coordination Supervisor, the Resource Coordinator is responsible for identifying, assessing, advocating, and providing assistance and information to individuals age 60 and their caregivers. This person serves as a single entry point for agency and community services and utilizes skilled interviewing techniques to assess client needs.
POSITION RESPONSIBILITIES
#183; Maintain and continually update resource manual to include information pertinent to persons 60 and their caregivers.
#183; Accept telephone calls, walk-in visitors, and client inquiries regarding services and resources available to persons 60 and their caregivers.
#183; Advocate for and assist persons 60 and their caregivers in securing needed agency and community services.
#183; Appropriately refer clients and/or caregivers to proper persons or agencies in the community, including Adult Protective Services.
#183; Utilize interviewing skills to complete comprehensive screenings of persons 60 for home delivered meals, personal care, free wills, and community programs.
#183; Promptly address a variety of emergency and elder abuse prevention assistance requests.
#183; Perform community presentations about agency services as requested.
#183; Attend community meetings and trainings.
#183; Maintain current and accurate written and electronic client records and prepare reports as required.
#183; Prepare for and pass AIRS (Alliance of Information and Referral Systems) exam in order to obtain certification as an Informational and Referral Specialist in Aging Services.
#183; Attend departmental staff meetings and other agency activities as requested. Perform other tasks related to the position.
ESSENTIAL SKILLS AND EXPERIENCE
#183; Education: Degree from an accredited college or university in social work or related field, or equivalent experience.
#183; Minimum of one year experience working with 60 population.
#183; Skilled interviewing techniques.
#183; Excellent oral and written communication skills.
#183; Comprehensive working knowledge of new and ongoing programs and services available to assist persons 60 and their caregivers.
#183; Knowledge of changing eligibility requirements for these programs.
#183; Ability to establish and maintain effective working relationships with clients, other professionals, staff, community agencies and the general public.
#183; Ability to prepare reports, maintain accurate records and enter client data into agency database.
#183; Ability to communicate effectively both orally and in writing.
#183; Ability to work independently.
SPECIAL REQUIREMENTS
#183; Driver’s license valid in the Commonwealth of Virginia.
#183; Virginia Notary Public Commission helpful.
