Position Summary:

FACETS is seeking a passionate and mission-driven Hypothermia Prevention Coordinator/Case Management Supervisor to join the team at Bailey’s Community Shelter. FACETS is a non-profit organization that opens doors by helping parents, their children, and individuals who suffer the effects of poverty in Fairfax County. We meet their emergency needs through our wrap around services and community partnerships and help individuals and families experience safe and stable housing after years of uncertainty and instability. Duties and Responsibilities:

  • Oversee daily operations of the shelter program, including logistics, staffing coverage, shift activities, and site preparation. Ensure low-barrier access is maintained at all program sites to support equitable entry and service delivery.
  • Assist with the recruitment of site hosts and collaborate with community groups to coordinate meal provision, rotating faith site schedules, and drop-in center duties throughout the hypothermia season.
  • Train seasonal staff and site-based volunteers on health and safety policies, program procedures and trauma-informed engagement practices to maintain consistency and safety across all sites.
  • Facilitate monthly meetings with Points of Contact (POC) from host sites to address opportunities, challenges, and feedback. Lead weekly conference calls with host locations to provide guidance, feedback, support, and coordination.
  • Collaborates with the Fairfax County Health Department’s Homeless Healthcare Program and the Community Services Board’s PATH Team to coordinate access to physical and behavioral health services for individuals experiencing homelessness.
  • Provides direct supervision, guidance and training to Hypothermia staff and Outreach Case Managers to ensure effective service delivery and professional development.
  • Monitors case management caseloads and assign cases to Case Managers to ensure balanced caseloads and effective client support.
  • Assesses the housing barriers of individuals experiencing or at risk of homelessness to identify their housing and service needs.
  • Maintains consistent verbal and written communication with case managers, operations staff, tenants, landlords, referral sources, collaborating agencies, debtors, and creditors to ensure effective coordination and support.
  • Advocates for and assist clients in accessing a range of services, including mental health and substance use treatment, housing referrals, financial assistance, home-based support, training, medical care, mentoring, and socialization opportunities.
  • Work on an adjusted work schedule, including evenings and weekends, as necessary to meet program needs.
  • Performs other duties as assigned.


Required Skills/Abilities:

  • Bachelor’s degree in social work, human services, psychology, or related field preferred.
  • Experience working with individuals experiencing homelessness.
  • Knowledge of trauma-informed care, harm reduction, and Housing First principles.
  • Strong written and verbal communication, crisis intervention, and de-escalation skills.
  • Excellent organizational and time management skills.
  • Familiarity with Homeless Management Information System (HMIS).
  • Proficient with Microsoft Office Suite or related software, including Microsoft Excel
  • Valid driver’s license, reliable transportation, good driving record, and personal car insurance.
  • Ability to drive a 15-passenger van.
  • Must pass criminal background checks.

Physical Requirements:

  • Prolonged periods of sitting at a desk and working on a computer.
  • Must be able to lift 15 pounds at times.
  • Must be able to attend occasional meetings at other FACETS office locations within Fairfax County, including 3 annual organization-wide fundraising events each year.

Salary Range:

$60,000 - $65,000 annually
