Responsibilities

About the Role

As a Provider, you will conduct in-depth health assessments tailored to each member’s needs during visits that may last up to an hour. Your role includes:

• Performing thorough adult and geriatric health assessments, including medical history, diagnosis, and treatment planning.
• Administering and ordering necessary point-of-care and lab-based tests.
• Collaborating with a team of healthcare professionals, including physicians, social workers, and family members, to ensure comprehensive care.
• Providing education and follow-up in cooperation with the member’s Primary Care Physician (PCP).
• Meeting members where they are, either in their homes or Skilled Nursing Facilities.

Qualifications

Skills & Experience:

• Master's Degree or equivalent experience, with NP licensure.
• Current NP licensure with prescriptive authority or ability to obtain.
• Certification from AANP, AACN, or ANCC in relevant specialty areas.
• 1 year of NP experience is preferred, though new graduates are encouraged to apply.
• Valid state driver's license and automobile insurance meeting local requirements.
• Strong computer skills and familiarity with medical record software.
• Excellent verbal and written communication skills.
