Resource Specialist
Reports to: VP of Care Coordination

GENERAL PURPOSE
Under the supervision of the Care Coordination Supervisor, the Resource Coordinator is responsible for identifying, assessing, advocating, and providing assistance and information to individuals age 60+ and their caregivers. This person serves as a single entry point for agency and community services and utilizes skilled interviewing techniques to assess client needs.
POSITION RESPONSIBILITIES
· Maintain and continually update resource manual to include information pertinent to persons 60+ and their caregivers.
· Accept telephone calls, walk-in visitors, and client inquiries regarding services and resources available to persons 60+ and their caregivers.
· Advocate for and assist persons 60+ and their caregivers in securing needed agency and community services.
· Appropriately refer clients and/or caregivers to proper persons or agencies in the community, including Adult Protective Services.
· Utilize interviewing skills to complete comprehensive screenings of persons 60+ for home delivered meals, personal care, free wills, and community programs.
· Promptly address a variety of emergency and elder abuse prevention assistance requests.
· Perform community presentations about agency services as requested.
· Attend community meetings and trainings.
· Maintain current and accurate written and electronic client records and prepare reports as required.
· Prepare for and pass AIRS (Alliance of Information and Referral Systems) exam in order to obtain certification as an Informational and Referral Specialist in Aging Services.
· Attend departmental staff meetings and other agency activities as requested. Perform other tasks related to the position.
ESSENTIAL SKILLS AND EXPERIENCE
· Education: Degree from an accredited college or university in social work or related field, or equivalent experience.
· Minimum of one year experience working with 60+ population.
· Skilled interviewing techniques.
· Excellent oral and written communication skills.
· Comprehensive working knowledge of new and ongoing programs and services available to assist persons 60+ and their caregivers.
· Knowledge of changing eligibility requirements for these programs.
· Ability to establish and maintain effective working relationships with clients, other professionals, staff, community agencies and the general public.
· Ability to prepare reports, maintain accurate records and enter client data into agency database.
· Ability to communicate effectively both orally and in writing.
· Ability to work independently.
SPECIAL REQUIREMENTS
· Driver’s license valid in the Commonwealth of Virginia.
· Virginia Notary Public Commission helpful.
