About the Role
In this role, you will conduct thorough health assessments that provide essential insights into our members' health. Your responsibilities will include:
- Performing comprehensive adult and geriatric assessments involving detailed medical histories, diagnostics, health education, case management referrals, and meticulous documentation following our professional standards.
- Ordering and administering necessary point-of-care and laboratory tests.
- Collaborating with physicians, case managers, social workers, family caregivers, and other healthcare professionals to ensure a holistic approach to care.
- Working closely with Primary Care Physicians to enhance patient education and ensure proper follow-up.
- Delivering care in the members' residences, including their homes or Skilled Nursing Facilities.

- Work-Life Balance: Our assessments are conducted seven days a week from 8 am to 7 pm, but typical full-time providers enjoy a Monday to Friday schedule from 8:00 am to 5:30 pm.
- Competitive Compensation: Receive an annual base salary ranging from $95,500 to $158,400, depending on your experience and market analysis.

- Licensure Support: When necessary, we provide assistance in obtaining additional state licensure and credentialing.
Qualifications:
- A Master's Degree or equivalent experience, along with successful completion of NP or PA licensure requirements.
- Current NP or PA licensure with prescription authority in the applicable state, or the capability to obtain it as needed.
- Board certification by NCCPA, AANP, AACN, or ANCC in relevant specializations.
- Up-to-date BLS, ACLS, or CPR certification.
- A minimum of one year NP or PA experience is preferred; however, new graduates are encouraged to apply!
- Willingness to travel up to 15%, including the possibility of overnight trips.
- A valid driver's license and proof of adequate auto insurance are mandatory.
- Strong computer skills and proficiency with health/medical record software are essential.
- Exceptional verbal and written communication skills are crucial.
