About the Position

VCU Health is seeking an experienced RN for their innovative Hospital at Home program. This role provides acute-level "inpatient" care directly in the patient’s home environment. Patients are stable MedSurg-level individuals who are monitored 24/7 virtually via telehealth and require twice-daily physical visits from the Hospital at Home RN.

Key Responsibilities

• Acute Home Care: Deliver therapeutic interventions, clinical assessments, and medication management typically found in an inpatient setting within a patient's living space.
• Technology Integration: Utilize remote clinical care coordination and telehealth technology to monitor patients.
• Team Nursing: Work as part of a multi-disciplinary healthcare team to maintain high quality and safety standards for the "Hospital at Home" census (typically 10–12 patients).
• Visits: Conduct twice-daily physical visits to assigned patients to implement individualized plans of care.

Schedule & Environment

• Shifts: 8 or 12-hour shifts (07:00–19:00 or 11:00–23:00)
• Rotation: Includes weekends and holidays.
• Orientation: 16 hours.
• Location: Based out of 1250 E Marshall St, Richmond, VA, servicing the surrounding community.

Requirements

Experience & Education

• Clinical Experience: 3 years of combined experience in Acute Care, MedSurg, Telemetry, or Critical Care is required.
• Education: BSN required .
• Preferred Experience: Trauma Level I, Telephone Triage, and previous Home Health experience.
• Travel Experience: Not required (First-time travelers accepted).

Certifications & Licensure

• Active Virginia (VA) or Compact RN License (Must be in hand at time of submission).
• Current BLS (AHA).
