Develop and implement individualized, evidence-based plans of care that emphasize functional restoration, mobility training, strength, endurance, balance, gait optimization, and ergonomic education. Apply manual therapy, therapeutic exercises, neuro-musculoskeletal techniques, and aerobic conditioning as appropriate. Monitor patient progress through regular re-evaluations, adjust treatment plans, document progress notes accurately, and communicate effectively with interdisciplinary teams to optimize discharge planning and continuity of care. Provide patient and caregiver education on home programs, safety, fall prevention, energy conservation, and strategies for long-term wellness.
