We are seeking a dedicated and license Speech-Language Pathologist (SLP) to join our home health team. The SLP will evaluate, diagnose, and treat patients with speech, language, voice, cognitive-communication, and swallowing disorders. This role is vital in helping clients regain communication and functional independence in their daily lives.
Duties and Responsibilities:
- Make home visits to clients in designated geographic territories
- Perform thorough evaluations to assess, diagnose, and provide therapeutic interventions for clients who are experiencing speech, language, communication, or swallowing difficulties
- Develop a treatment plan based on the evaluation, physicians orders, and recommended goals and revise the speech therapy care plan
- Continually assess and revise the speech therapy care plan, and participate with clinicians in a multidisciplinary care team (PTs, Nurses, Speech Therapists), as appropriate to ensure comprehensive client care
- Educate and instruct clients, family members, caregivers, or other patient representatives in rehabilitative care and activities necessary to promote the clients health, safety, and independent living
- Monitor and document client progress, treatment plans, and goals accurately and timely
- Ensure compliance with all State, Federal documentation, regulatory standards, and ethical practices as well as our agencys policy and procedure for home healthcare services
- Participate in case conferences, patient care planning, and interdisciplinary team meetings
- Participate in continuous education and training to stay updated with the latest physical therapy practices.
Qualifications:
- Bachelors or Masters degree in Speech-Language Pathology from an accredited educational institution
- Must hold a valid license as a Speech-Language Pathologist from the State of Virginia
- Certificate of Clinical Competence (CCC-SLP) from ASHA preferred
- Prior experience in a home healthcare setting is preferred but not required
- Excellent communication and interpersonal skills
- Ability to work independently and collaboratively in a team environment
- Strong problem-solving skills and attention to detail
- CPR/BLS certification is required
- Must be a licensed driver with an automobile in good working condition
- Professional Liability Insurance or be willing to purchase if applicant is interested in 1099 employment vs W2

Job Type : Part Time and Full Time roles Available
Pay Rate : $50.00 $80.00/hr and/or $80.00 $120.00 per visit. Rates are negotiable based on applicants experience
