RN Case Manager Job Details

• Location: Charlottesville, VA 22908 (Virginia nursing jobs)
• Healthcare Setting: Acute Care Hospital – Case Management Unit
• Assignment Length: 13 weeks (September 15, 2025 – December 15, 2025)
• Schedule: Day shift, 5x8-hour shifts per week (08:00 – 16:00)
• Hours per Week: 40
• Weekend Coverage: 2-3 weekends during contract (every 4th weekend)
• Patient Population: Adults and Geriatrics
• Estimated Weekly Salary: $2050 - $2100

Registered Nurse Case Manager Qualifications

• Active Registered Nurse (RN) license in Virginia (pending accepted, must be in-hand before start date)
• Minimum 2 years of acute care hospital inpatient case management experience
• Experience with Epic charting system or previous travel RN experience (must have at least one)
• BLS certification (American Heart Association)
• BSN preferred
• Strong skills in care coordination, discharge planning, disease management, needs assessment, and plan of care development
• Ability to float within scope of practice as needed
• Flu vaccine required (no exemptions)
• References required

RN Case Manager Responsibilities

• Coordinate and manage patient care transitions, including discharge planning and needs assessments
• Collaborate with interdisciplinary healthcare teams to ensure optimal patient outcomes
• Utilize Epic for clinical documentation and care planning
• Manage a caseload of approximately 30 patients during day shifts
• Provide disease management education and support to patients and families
• Order DME and coordinate post-discharge services as needed
• Participate in weekend coverage as scheduled
• Maintain compliance with facility policies and regulatory requirements
