Job Title: Registered Nurse Profession: Nursing Specialty: Home Health Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (08:00 - 16:00) Experience: Minimum 2 years of home health experience License: Valid driver's license and clean driving record required Certifications: BLS required Must-Have: Must have home health and OASIS experience. Local candidates are accepted at the same rate. Charting experience in EPIC is required. Description: The position requires a registered nurse with home health experience. Candidates must be able to see six visits per day or thirty visits per week. Daily census averages around four hundred thirty patients. This role involves various visit types, including initial evaluations, recertifications, and follow-ups. Mandatory patient demographics include adults and geriatrics. The workload includes specific point values based on visit types, with expectations of six points per day and rotation every fourth weekend. Interdisciplinary support from physical therapy and social services is available. Candidates must possess skills in assessment, care planning, and patient education.
