Job Summary
We are looking for individuals to join our client’s team as Licensed Master Social Worker. You will support patients’ psychosocial needs, discharge planning, and coordination of care within a multidisciplinary team.
Job Description
- Conduct thorough biopsychosocial assessments and develop appropriate intervention plans for patients in inpatient medicine, surgery, and rehabilitation settings, in accordance with hospital and departmental policies.
- Provide ongoing supportive counseling and psychosocial interventions to patients and their families/significant others.
- Communicate with patients’ families, collaterals, community providers, and medical teams; incorporate this information into assessments, documentation, and care planning meetings.
- Coordinate with community resources to support patient discharge and successful reintegration into home or community settings.
- Attend multidisciplinary team meetings and rounds to contribute to care planning.
- Advocate for patients and collaborate with social and community agencies to ensure access to necessary services.
- Maintain accurate and timely documentation according to departmental and hospital standards.
- Provide caring, ethical, and nonjudgmental services to all patients.
- Participate in performance improvement initiatives as required.
- Follow hospital policies for universal abuse screening (child, intimate partner, elder, sexual assault, human trafficking) and NYS mandated reporting protocols.
- Participate in regular supervision and escalate issues as necessary.
- Fulfill essential worker responsibilities, including reporting to work during inclement weather, pandemics, or other emergency situations, per hospital policy.
Skills Required
- Experience in inpatient medicine, surgery, or rehabilitation (med/surg).
- Someone with Hospital experience
- At least 1 year of experience
Education/Training/Certifications
- Required Education: Master’s degree in social work
- Valid Licensed Master of Social Work (LMSW) license issued by NYSED required
Additional Requirements
- Schedule notes: 9:00 AM-5:00 PM, M-F; 35 hours per week (1 hour lunch)
Pay Rate
- $52–$61 per hour, based on experience, skills, and market factors
