Harmonious Care Services is a home healthcare agency dedicated to delivering high-quality, compassionate, and personalized home healthcare services that promotes independence and well-being. Our interdisciplinary team works collaboratively to ensure every patient receives personalized, effective care in the comfort of their home.

We are seeking a skilled and compassionate Occupational Therapist (OT) to join our home health team. The OT will be responsible for assessing patients functional abilities and developing treatment plans to help them regain independence in daily activities. This role involves close collaboration with patients, families, and other healthcare providers to ensure comprehensive, goal-oriented care.

Duties and Responsibilities:

- Evaluate patients physical, emotional, and cognitive abilities related to daily living and working activities

- Develop and implement individualized care plans to improve patients ability to perform daily tasks

- Demonstrate expertise in using treatment modalities, equipment, and protocols

- Perform re-evaluations and adjust treatment plans as needed

- Educate patients and caregivers on adaptive techniques, equipment use, and home modifications

- Monitor and document patient progress, treatment plans, and goals accurately and timely using Electronic Medical Records (EMR) tools like AloraPlus

- Collaborate with other healthcare professionals (PTs, Nurses, Speech Therapists) to ensure comprehensive client care

- Communicate effectively with clients, their families or caregivers, medical staff, and care team members

- Ensure compliance with all State, Federal documentation, regulatory standards, and ethical practices as well as our agencys policy and procedure for home healthcare services

- Participate in case conferences, patient care planning, and interdisciplinary team meetings

- Participate in continuous education and training to stay updated with the latest physical therapy practices.

Qualifications:

- Bachelors or Masters degree in Occupational Therapy from an accredited educational institution

- Must hold a valid license as an Occupational Therapist from the State of Virginia

- Prior experience in a home healthcare setting is preferred but not required

- Excellent communication and interpersonal skills

- Ability to work independently and collaboratively in a team environment

- Strong problem-solving skills and attention to detail

- CPR/BLS certification is required

- Professional Liability Insurance or be willing to purchase if applicant is interested in 1099 employment vs W2

- Must be a licensed driver with an automobile in good working condition

Job Type: Part Time and Full Time roles Available

Pay Rate: $45.00 $80.00/hr and/or $100.00 $140.00 per visit. Rates are negotiable based on applicants experience
