Responsibilities
About the Role
As a Matrix provider, you will conduct comprehensive assessments, lasting up to one hour, to gather insights about a member's health status, medical history, and social context. This thorough evaluation aims to bridge care gaps and improve health outcomes. Your responsibilities will include:
- Performing detailed adult/geriatric assessments, evaluating medical history, diagnosis, treatment plans, health education, referrals, and follow-ups.
- Ordering and administering necessary point-of-care and laboratory tests.
- Collaborating with physicians, case managers, social workers, caregivers, and other medical professionals.
- Partnering with Primary Care Physicians (PCPs) to inform patients and coordinate follow-up care.
- Providing in-home services or assessments in Skilled Nursing Facilities.

Qualifications
Skills & Experience That Shine
- Master's Degree or relevant experience, along with successful NP or PA licensure.
- Current NP or PA licensure with prescriptive authority in your state, or the ability to secure it.
- Board certification from NCCPA, AANP, ANCC, or AACCN in applicable specialties.
- Current BLS, ACLS, or CPR certification.
- Preferred: 1 year of experience as a Nurse Practitioner or Physician Assistant; new graduates are also encouraged to apply!
- Valid state driver's license and adequate automobile insurance coverage.
- Proficient computer skills and familiarity with healthcare software.
- Excellent verbal and written communication abilities.
