Job Overview
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our dynamic healthcare team as a Staff Nurse. In this vital role, you will provide high-quality patient care across various settings, including acute care, long-term care, assisted living, and community health environments. Your expertise will support patient recovery, comfort, and well-being while ensuring compliance with healthcare standards and regulations. Duties
- Perform comprehensive patient assessments, including vital signs, physical examinations, and physiological evaluations to inform care plans.
- Administer medications and injections accurately following prescribed protocols, including IV infusions and medication management.
- Assist with medical procedures such as catheterization, suturing, tube feeding, and sterile processing techniques.
- Monitor patients' conditions continuously, recognize signs of acute pain or deterioration, and respond promptly to emergencies such as airway management or ventilator issues.
- Document all patient interactions meticulously using Electronic Health Record (EHR) systems like Epic or Cerner, ensuring compliance with HIPAA regulations.
- Collaborate with physicians, case managers, and interdisciplinary teams to develop discharge plans and coordinate post-acute or hospice care.
- Provide specialized care for diverse populations including geriatrics, pediatric patients, individuals with developmental disabilities, Alzheimer's disease, dementia, or those requiring memory or behavioral health support.
- Support infection control practices and maintain sterile techniques during procedures such as phlebotomy or specimen collection.
- Educate patients and families on health coaching topics like medication administration, nutrition, disease management (e.g., infectious diseases), and home & community care strategies.
Qualifications
- Valid Licensed Practical Nurse (LPN) license in good standing; additional certification in Basic Life Support (BLS) required.
- Proven experience in various healthcare settings such as hospital medicine, medical-surgical units, ICU/CCU/Trauma Centers, emergency rooms, or long-term care facilities.
- Demonstrated proficiency with EMR/EHR systems including Epic, Cerner, Athenahealth or eClinicalWorks.
- Strong knowledge of medical terminology, anatomy, physiology, ICD coding (ICD-9/10), CPT coding for procedures like spinal taps or gastric lavage.
- Experience with IV insertion/insertion techniques including IV infusion therapy; familiar with ventilator management and airway procedures.
- Skilled in patient assessment techniques such as vital signs monitoring and diagnostic evaluation; capable of performing physical examinations confidently.
- Ability to work effectively with diverse populations including children (pediatric care), seniors (geriatrics), individuals with disabilities or dementia; experience in hospice or palliative care is a plus.
- Excellent communication skills for collaborating with multidisciplinary teams and educating patients/families on complex health topics.
- Knowledge of infection control standards and aseptic techniques; familiarity with ICD coding standards for accurate medical documentation.
