The REACH Coordinator, works with individuals with developmental disabilities and behavioral health needs, their families/care givers, and support providers, and community partners to strengthen treatment/support efficiencies and service outcomes. The REACH Coordinator serves as a vital community education resource to the community and coordinates admission into REACH services.


Starting Salary: $48,208-Depending on Experience

Responsibility
  • Develops and updates as needed comprehensive service evaluations, crisis education prevention plans, intake/assessments, intervention and outcomes plans for REACH Crisis Therapeutic Home (CTH) admissions, and any other applicable documentation as assigned.
  • Works with clinical staff on REACH team to ensure completion of required assessment of needs and strengths to meet the admission and ongoing provision requirements for REACH services. Ensures the proper pre-authorization documentation to qualify for and receive payment for all services rendered are submitted within allowable time frames and maintained in the record.
  • Writes effective and measurable individualized service plans as required upon admission, as the needs of individuals served change, and at least once annually thereafter. Works to ensure supports and interventions are provided as stated in the plan. Ensures all assigned plans are reviewed at least quarterly.
  • Maintains primary responsibility for coordinating REACH support meetings and serves as the lead staff person on the team to ensure all services and supports are provided as outlined in the service plans.
  • Works closely with Emergency Services' staff and other Community Services Board providers to assists with admission process and discharge planning to and from private and public psychiatric hospitals and training centers, including providing support and input to the emergency services providers and judicial system during commitment hearings, when appropriate.
  • Serves as on-call shift leader before and after hours as scheduled. Provides back-up for co-workers as needed. Provides after-hours emergency crisis stabilization services as needed. Must be accessible by telephone after hours.
  • Maintains complete and accurate medical records, and completes all administrative documentation according to policy and procedures
  • Collaborates with other agencies and NRVCS programs to ensure that all services reflect the individuals' needs. Monitors satisfaction with services provided and helps individuals served complete satisfaction surveys as needed
  • Promotes independence by teaching and modeling life skills such as: personal hygiene, nutrition, understanding of mental and physical health and well-being, methods to access assistance, financial management, coping skills, self-esteem and self-reliance.
  • Assists with crisis resolution and general problem-solving. Ensures services are provided with respect to the dignity of the individuals served as outlined in the State and NRVCS Human Rights Plans. Advocates for services on behalf of individuals served and their families/care givers as needed. Documents and reports suspected violations of Human Rights according to NRVCS policies.
  • Ensures all safety, health, and universal precautions are properly addressed and monitored. Completes NRVCS Incident Report forms by the end of the shift for any observed or suspected incidents that involve individuals served or staff welfare and/or safety.
  • Provides psycho-education to individuals served and families/care givers to promote better understanding of behavioral health related topics. Works to establish linkages between individuals served and naturally occurring supports in their family and community. Participates in on-call rotation.
  • Promotes positive communication and collaboration among team members and other community resources needed to provide quality services. Conducts self in a professional manner at all times. Participates in team meetings unless absence is excused, completes proper written communication and relays other helpful information as necessary by voice mail and/or e-mail. Serves on designated teams, committees and work groups.
  • Maintains all communications, both written and verbal, with respect and confidentiality and complies with all HIPAA and 42 CFR Part 2 standards and regulations.
  • Performs at the expected levels of individual and team productivity. Ensures all client records are in compliance with applicable local, state, and federal requirements including guidelines as they pertain to DBHDS Licensure Regulations, Waiver, State Plan and Clinic Options Medicaid.
  • Ensures all required certifications and/or licensure are kept up to date. Attends/provides other training as required to meet personal professional development and/or program needs.
  • Completes other related duties as assigned by Supervisor, Program Manager, Program Director and/or Senior Director.
Qualifications
  • Extensive knowledge and training in best practices for providing community-based crisis services for individuals with co-occurring developmental disabilities and behavioral health diagnoses residing in community settings.
  • Demonstrated knowledge of agency, state, and federal regulations and standards for REACH, including Medicaid and accreditation standards.
  • Ability to communicate effectively orally and in writing and to prepare and present educational materials.
  • Must possess a valid driver's license (Virginia, unless otherwise approved by agency) and an acceptable driving record must be maintained throughout employment.
  • Acceptable criminal history record and child protective services check upon hire and must be maintained throughout employment.
  • Acceptable drug screen results upon hire and as required by policy for position.
Education/Experience Required:

  • Graduation from an accredited university with at least a Master's degree in a human services related field
  • Two years of experience working with individuals with developmental disabilities and behavioral health
  • OR a bachelor's degree and five years of experience working with individuals with developmental disabilities and behavioral health diagnoses is required.
  • QMHP A and/or C required.
Additional compensation will be considered for any new employees who speak multiple languages.
