Job Summary

We are looking for individuals to join our client’s team as Case Worker. You will be responsible for conducting interviews, performing initial assessments, and making appropriate referrals to support services.

Job Description

  • Conduct comprehensive assessments of patients’ social, emotional, and medical needs.
  • Develop and implement individualized care plans, ensuring follow-up and continuity of care.
  • Connect patients with community resources such as housing, food assistance, mental health services, and public benefits programs.
  • Request and process referrals, including placement of patients into nursing homes and post-acute care facilities.
  • Utilize Careport and EPIC (preferred) for referrals, documentation, and care coordination.
  • Maintain accurate and timely documentation of case notes, client interactions, and progress reports in compliance with HIPAA and organizational policies.
  • Collaborate with interdisciplinary teams, including healthcare providers, social workers, and administrative staff.
  • Provide crisis intervention and support for patients experiencing urgent situations.
  • Educate patients and families on available services and resources.
  • Participate in staff meetings, training sessions, and continuous professional development.

 

Skills Required

  • Minimum of 3 years’ experience in social services, case management, or healthcare support.
  • Direct experience with nursing home placement and case management required.
  • Experience sending out referrals; Careport and EPIC knowledge preferred.
  • Strong documentation and reporting skills, with excellent attention to detail.
  • Excellent interpersonal, communication, and problem-solving skills.
  • Ability to work independently and collaboratively within a multidisciplinary team.

Education/Training/Certifications

  • Bachelor’s Degree required from an accredited college or university

Additional Requirements

  • Shift Time: 9:00 AM – 5:00 PM

Pay Rate

  • $43.00 per hour
