Physical Therapist Job Details

• Location: Campbell, CA and nearby counties (Santa Clara, Santa Cruz, Hollister)
• Assignment Type: Travel/Locum Tenens Physical Therapist
• Healthcare Setting: Home Health (hospital-based)
• Contract Duration: 13 weeks (September 1, 2025 – December 1, 2025)
• Schedule: Day shifts, Monday–Friday, 8:00 AM – 5:00 PM (5x8-hour shifts)
• Estimated Weekly Salary: $2390–$2460
• Productivity: 30 points per week (average 5–6 patients/day)
• Mileage: Reimbursed at IRS standard rate (travel up to 75 miles from home clinic)
• EMR System: HomeCare HomeBase (HCHB)

Physical Therapist Qualifications

• Minimum 1 year of recent adult Home Health physical therapy experience (first-time travelers not accepted)
• Active California Physical Therapist license (must have license in hand at time of submission)
• Current CPR/BLS certification
• Proficiency with HomeCare HomeBase EMR or willingness to retrain
• Reliable vehicle, valid driver’s license, and agency-required liability insurance
• Willingness to travel up to 75 miles from assigned care center
• Ability to complete all documentation within 24 hours of patient visit
• Open to on-call rotations and floating to nearby clinics as needed

Physical Therapist Responsibilities

• Deliver skilled physical therapy services to adult patients in their homes, including start of care and follow-up visits
• Manage a daily caseload of 5–10 patients, meeting productivity expectations
• Travel between patient homes and attend office meetings at the Campbell clinic
• Complete timely, accurate documentation using HomeCare HomeBase EMR
• Collaborate with interdisciplinary healthcare team members to develop and implement individualized care plans
• Educate patients and families on therapy goals, exercises, and safety protocols
• Maintain compliance with agency policies and regulatory requirements
