- Location: Virginia Beach, VA 23452 – 100% on-site, filed work/home health setting
- Covering : Chesapeake, Virginia Beach, Portsmouth, Suffolk, and Norfolk
- Payrate: $56.15/hr + $0.54/mile
- Required Availability: Full-Time | Monday – Friday, 8:00 AM – 5:00 PM
- This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs
- Benefits are available to full-time employees after 90 days of employment and include health, optical, dental, life, and short-term disability insurance
- A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates
- Comprehensive Assessment: Perform detailed assessments of high-risk members and identify opportunities for health promotion and illness prevention.
- Care Coordination: Collaborate with PCPs, social workers, and discharge planners to implement individualized care plans.
- Resource Management: Identify cost-efficient levels of care and arrange non-medical supports, such as housing and transportation, to ensure treatment compliance.
- Patient Advocacy: Protect patient privacy and confidentiality while intervening quickly to minimize poor clinical outcomes.
- Documentation: Maintain precise records of clinical, functional, and fiscal outcomes throughout the management process.
- Education: Associate’s or Bachelor’s Degree in Nursing (or a related field).
- Attendance: Commitment to mandatory attendance during the first 90 days.
- Licensure: Must hold a current, unrestricted Registered Nurse (RN) license in Virginia.
- Certification: Case Management certification is required.
- Clinical Skills: Ability to analyze complex medical information and make rational, evidence-based decisions.
- Preferred Skills: Oasis assessments, Wound care and Wound Vac, IV therapy and lab draws, Chronic disease management
