Title: Rehab Team Member - Rehabilitation

Schedule:

Requirements:
- Experience in a skilled nursing facility (SNF) setting
- Proficient in modalities, ADL training, falls and balance, dysphagia management, vital stim, contracture management, and splinting
- Familiarity with E-Docs via Net Health

Roles & Responsibilities:
- Provide rehabilitation services to patients in a skilled nursing facility setting
- Collaborate with a cohesive rehab team to implement individualized treatment plans
- Assist patients with activities of daily living (ADLs) and mobility training
- Document patient progress and treatment outcomes using electronic health records
- Participate in educational opportunities for continued professional development

Compliance / Notes:
- 10% salary placement fee
- Education opportunities via Med Bridge for licensure renewals
- No weekend rotation
- Parking and dress code policies may apply
