Nurse Practitioner-Weekends - Hampton, VA
About the Role
We are seeking a compassionate and experienced Nurse Practitioner to provide high-quality clinical care across weekend shifts in Hampton, VA. This position offers the opportunity to deliver autonomous, patient-centered care within post-acute and long-term care environments while working closely with multidisciplinary facility teams.
Schedule
This role covers weekend shifts (Saturday and Sunday). Flexible shift patterns may be available based on coverage needs and candidate preferences.
Core Responsibilities
Provide comprehensive medical care to residents in post-acute, long-term care, and assisted living settings during weekend coverage
Perform thorough assessments, develop and implement evidence-based treatment plans, and manage acute and chronic conditions
Prescribe medications and manage medication adjustments within scope of practice and Virginia regulations
Order and interpret diagnostic tests, and coordinate follow-up care with specialists and primary teams as required
Document patient encounters accurately and promptly in the Electronic Medical Record (EMR)
Collaborate with nursing staff, physicians, therapists, and other allied health professionals to ensure continuity of care
Participate in care planning, case reviews, and quality improvement initiatives
Provide clear, compassionate communication to families and facility staff regarding clinical status and care plans
Qualifications and Required Skills
Master’s or Doctoral degree as a Nurse Practitioner from an accredited institution
Active, unrestricted Virginia NP license and current DEA registration
Current certifications as required by role and facility policies (e.g., BLS)
Experience in post-acute care, long-term care, geriatrics, or related settings preferred
Proficiency with Electronic Medical Record systems
Strong clinical judgement, excellent communication skills, and the ability to work independently during weekend shifts
Compassionate, patient-centered approach and ability to build professional relationships with multidisciplinary teams

Maintaining clear, comprehensive documentation in the EMR
Fostering collaborative relationships with facility staff and families to support patient-centered outcomes
