Job Description
Provide the necessary mental health treatment and support necessary to prevent homelessness and gain independent living skills. QMHPs are expected to model through their behaviors and attitudes the values of JOURNEY COMMUNITY CARE, LLC.
limited interventions provided typically, but not solely, in the home of adults with serious mental illness as described by regulations to address independent living skills.
Duties and Responsibilities:
The QMHP is responsible for services primarily to be provided via face-to-face involvement with the individual and other family members. Services are primarily within the home although they may occur in various community settings. The QMHP is required to assist in the development and implementation of the person-center, recovery-based treatment plan. The activities associated with the primary responsibility of this Qualified Mental Health Professional (QMHP) are:
● Develop and maintain Individual Service Plans and Quarterly reports with individuals.
● Work with individuals to foster new or improved independent living skills.
● Case management functions of linking, arranging, and advocating for services and referrals.
● Maintain face-to-face contact with individuals and their support system 6-15 hours per week per individual.
● Maintain up-to-date individual records and prepare required session notes in a timely and accurate manner.
● Establish a good working relationship with other public and private agencies/services and interface collaboratively with them.
● Reduce the risk of out-of-home placement or aid in return from out-of-home placement.
● Attend monthly staff meetings and supervision.
● Respond promptly to emails and telecommunication.
● Ongoing assessment of treatment needs.
● Complete quarterlies and continued stay requests
● Work with individuals to foster the development of new or improved independent living skills
● Maintain continuous contact with the identified individual and other key stakeholders (with a signed authorization to release information) in planning and service delivery. These stakeholders might include but are not limited to social workers, psychiatrists, court officials and police, teachers, and other human service professionals.
● Educate individuals and family members regarding existing community services, and act as an advocate in connecting them to these services.
● Respond to family crises on a 24-hour, seven-days-a-week basis.
● Maintain up-to-date individual records and prepare required/necessary reports in a timely and accurate manner.
● Attend court when necessary.
● Establish a good working relationship with other public and private agencies/services and interface collaboratively with them.
Academic/Work Experience:
Qualified Bachelor's degree and must qualify as a Qualified Mental Health Professional (QMHP-A) through the Board of Counseling and maintain credentials as directed by the Board. No other exceptions will be allowed.
