The Youth Mentor working for the Kids Program for Assertive Community Treatment (KPACT), provides behavioral management support, social skill development, intensive supervision, crisis stabilization services, transportation, and family support to children and adolescents with mental illness, substance use disorders, and/or developmental disabilities in the home, school, and community settings. Assists with the implementation of the client's and family's treatment plan by facilitating treatment/behavioral strategies to assist with the achievement of identified goals. Develops and maintains working relationships with: individuals served, families and/or caregivers, other NRVCS programs, and other community agencies and resources as needed to ensure quality services. Works as part of the KPACT treatment team to ensure services and resources are coordinated among providers as needed to help individuals served to live and function optimally in the community.


Starting Hourly Rate: $16.05-Depending on Experience


Key Responsibilities:

  • Implements behavioral management strategies; facilitates activities designed to improve social and problem-solving skills; assists with crisis resolution; implements interventions to improve overall functioning as indicated by the KPACT treatment plan and as directed by the treatment team.

  • Provides direct and related services needed to ensure client needs are addressed. Performs or assists with intake, crisis resolution, and/or obtaining additional emergency services as needed and works as part of a team to ensure client services are consistent, appropriate, and accurate according to Family Assessment and Planning Team (FAPT) authorizations.

  • Meets routinely with supervisor to review the status of treatment and addresses any issues significant to the successful treatment of clients assigned.

  • Maintains complete and accurate program and client records and completes all paperwork within the required timeframe (i.e. progress notes,time sheets, mileage forms, incident reports,leave requests, etc.)

  • Establishes and maintains community referral network and advocates for services on behalf of consumers and their families including, but not limited to, educational, legal, medical, and financial assistance.

  • Collaborates and coordinates with Behavioral Support Providers and Family Clinicians on a regular basis.

  • Assists with crisis resolution and general problem-solving. Ensures services are provided with respect to the dignity of the individuals served as outlined in the State and NRVCS Human Rights Plans. Advocates for services on behalf of individuals served and their families/caregivers as needed. Documents and reports suspected violations of Human Rights according to NRVCS policies.
  • Promotes positive communication and collaboration among team members and other community resources needed to provide quality services. Always conducts self in a professional manner. Participates in team meetings unless absence is excused, completes proper written communication and relays other helpful information as necessary by voice mail and/or email.

  • Maintains all communications written or verbal with respect and confidentiality and complies with HIPAA and 42-CFR Part 2 standards and regulations.

  • Performs at the expected levels of individual and team productivity. Ensures all client records follow all applicable local, state, and federal requirements including guidelines as they pertain to DBHDS Licensure Regulations, Waiver, State Plan, and Clinic Options Medicaid.

  • Ensures that all training, certifications, and pertinent licenses for self are current and updated regularly.

  • Completes other related tasks as assigned by Program Manager,Director, and/or Senio rDirector.
Qualifications:

  • Knowledge and understanding of mental illness,substance use disorders, and co-occurring MH/SA treatment modalities as well as knowledge of the use of psychotropic medications.
  • Ability to establish and maintain good rapport with clients.
  • Demonstrated ability to provide emergency and crisis services, to advocate for clients, and to develop and maintain working relationships within the agency and community and work as part of a multi-disciplinary treatment team.
  • Demonstrated ability to communicate clearly and concisely (written and oral).
  • Initiative, resourcefulness, and ability to work both independently and as part of a team. Ability to manage and respond to emergency situations and have working knowledge of Virginia Code as it applies to least restrictive treatment.
  • Must possess a valid driver's license (state of Virginia, unless otherwise approved by agency) with an acceptable driving record. Must maintain acceptable record throughout employment.
  • Must have an acceptable criminal history background and child protective services check upon hire and maintained throughout employment.
  • Must have acceptable drug screen results upon hire.
Education/Experience Required:

  • High school diploma required.
  • Bachelor's degree from an accredited college or university in human services, or a related field and at least one year of experience working with individuals with intellectual disabilities, mental health, and/or substance abuse preferred.
  • Experience working with adults, children, and families is preferred.
