Responsibilities
- Assess home health patients in stable and emergent situations to identify physical, psychosocial, and environmental needs.
- Develop, document, and evaluate the Plan of Care to ensure quality and continuity of care.
- Obtain data on physical, psychological, social, spiritual, and cultural factors and incorporate that data into the Plan of Care.
- Maintain up-to-date patient records, documenting visits, problems, plans, actions, and goals; reflect changes as they occur.
- Educate patients and families regarding care, medications, home safety, and related topics; use teaching methods appropriate to age, condition, culture, and learning readiness; promote self-care practices.
- Supervise and provide clinical direction to HHA/STNA/LVN to ensure quality and continuity of service; participate in on-call coverage as assigned.

Requirements
- Active Registered Nurse license in the state of practice (Required)
- CPR certification (Required)
- Valid driver’s license with reliable transportation and insurance (Required)
- 1-2 years RN clinical care experience; 2+ years home health experience (Required)
- Associate Degree in Nursing or Nursing Diploma (Required); BSN (Preferred)
- OASIS experience and Homecare Homebase software experience (Required)
- CHPN certification (Preferred)
