Occupational Summary

The Hospice Nurse Practitioner is a licensed advanced practice registered nurse (APRN) who provides comprehensive hospice services in accordance with the state Nurse Practice Act. Under the supervision of a collaborating physician, the Nurse Practitioner conducts patient visits to perform face-to-face assessments and evaluate ongoing hospice eligibility in collaboration with the interdisciplinary team.

Primary Responsibilities

• Conduct face-to-face assessments prior to the third and subsequent benefit periods to evaluate continued hospice eligibility.
• Provide clinical findings and recommendations to the Hospice Medical Director for certification and recertification decisions.
• Maintain up-to-date knowledge and compliance with Medicare, Medicaid, and state/federal hospice regulations.
• Ensure adherence to HIPAA and all confidentiality standards.
• Participate in quality assurance and performance improvement activities.
• Engage in professional development through continuing education, seminars, and in-service training.
• Accept and complete assigned duties in a timely and responsible manner.
• Other duties as assigned.

• Graduate of an accredited School of Nursing and Nurse Practitioner program.
• Master’s degree in Nursing.
• Minimum of one (1) year Nurse Practitioner experience.
• Minimum of one (1) year experience in hospice or palliative care.
• Certification from an approved advanced practice nursing board as required by state law.
• Current licensure as a Registered Nurse and Nurse Practitioner in the state of practice.
• Current CPR certification.
• Proficient in English (reading, writing, and speaking).
• Strong observational, communication, and problem-solving skills.
• Ability to meet deadlines and work independently.
• Capable of performing CPR and managing airway obstructions.
• Physically able to climb at least two flights of stairs and stand or walk for up to four hours.
• Valid driver’s license and proof of current vehicle insurance.
• Satisfactory motor vehicle driving record as required by the agency and insurance providers.
