Responsibilities
About the Role
As a member of our team, you will conduct comprehensive health assessments that provide valuable insights into a member's health status. Your responsibilities will include:
- Performing adult and geriatric assessments including medical history, diagnosis, treatment plans, health education, and follow-up referrals.
- Administering and ordering point-of-care and lab tests as necessary.
- Collaborating with a multidisciplinary team which includes physicians, case managers, and caregivers.
- Delivering care directly in members' homes or skilled nursing facilities.

- Competitive Pay: Earn a base salary between $95,500 - $158,400 plus uncapped monthly incentives.

- Additional Licensure: We support obtaining state licensure and credentialing as needed.
Qualifications
- Master's Degree or equivalent experience along with NP or PA licensure.
- Current NP/PA licensure with prescriptive authority.
- Board certification in relevant specialties.
- Current BLS, ACLS, or CPR certification.
- Experience as an NP or PA preferred; new graduates are welcome to apply!
- Willingness to travel up to 15%, including overnight stays.
- Strong communication skills and computer proficiency.
