Licensed Professional Nurse (LPN) Geriatric Transition Center

JOB SUMMARY :
The Licensed Professional Nurse (LPN) of Geriatric Transition Center ensures that Geriatric Transition Center individuals, with a mental, behavioral or substance abuse diagnosis, are medically delivered in compliance with all federal, state, local and Agency regulations, policies, and procedures. Works as a team member with the Geriatric Program Manager, Geriatric Residential Nurse Lead, and other team members to ensure that urgent as well as routine and preventative healthcare is provided for each individual.
ESSENTIAL FUNCTIONS :
- Provide medical care as ordered by a physician or nurse practitioner in the area of nursing services, including but not limited to catherization. Administer medications and treatments as ordered by a physician or nurse practitioner. Provide information to the physician/practitioner for all medical appointments, schedule appointments, and may attend specialty and/or routine consults which would enhance the quality of medical care for the individual. Ensure medication and treatment records accurately reflect current physician's orders including ordering/filling prescriptions, ensuring over-the-counter medications are available and date is current, ensuring first aid equipment and supplies are fully stocked in area(s) of responsibility. Maintain a professional and cooperative relationship with medical service providers in the community. Provide emergency medical treatment, as needed.
- Monitor the health status of individuals living in assigned services. Provide training to direct support professionals on general health-related issues such as infection control, and health issues related to a specific individual.
- Document pertinent contacts made on behalf of individuals and any new medical/health related information. Ensure documentation of vital signs, weight, intake/output, seizure activity, and response to medication/treatment for individuals assigned to their care.
- Attend meetings to receive information or to provide medical/health related information as indicated, for individuals living in assigned residence.
- Communicate recommendations, orders, or changes in plan of treatment from medical providers. Communicate changes to residential staff as needed and per Registered Nurse direction.
- Maintain communication with other service providers to ensure continuity of health services, timely identification health concerns, and satisfactory and timely resolution of mutual issues relative to individuals served.
- Provide direct care and direct support services to individuals of the center including but not limited to: Provide instruction and supervision of individuals in completion of personal care, daily living, and community activities. Assist individuals in the pursuit of personal interest in the community, as well as, at the residence, including religious functions. Provide crisis intervention support services to individuals as needed or assigned. Intervene and report any incidences or unusual occurrences as per policy/procedure. Ensure any immediate protective action is taken. Ensure adequate diet according to identified needs and/or requirements. Understand, identify, and advocate for individual rights. Model appropriate interaction with the residents, and provide a positive role model for all individuals. Maintain compliance with State and Federal regulations, as well as, with Agency program policies and procedures. Maintain an effective and positive relationship with family, correspondents, friends, service providers, and the community-at-large. Report to supervisor(s) any information pertinent to the needs of the individual or to the operation of the program.
- Participate in the Program Planning Team, including identifying, implementing, and documenting individual program plan. Assist in providing a safe, clean, comfortable, and therapeutic environment. Perform routine nursing care including, but not limited to implementation of nursing interventions, medication administration, prescribed treatments, specimen collection, taking and recording of vital signs, assisting with hygiene, and administering enteral nutrition. Effectively understands the use and maintenance of frequently used equipment such as automatic blood pressure monitors, pulse oximeters, ventilators, enteral feeding pumps, IV pumps, digital thermometers, otoscopes, electronic communication devices, orthotics, and wheelchairs. Accurately and thoroughly completes all required documentation including, but not limited to: daily Nursing Notes, Medication Administration Records, Change of Condition reports, Seizure Reports, and Incident Reports. Monitor, promote, and participate in the implementation of Active Treatment. Work collaboratively with Respiratory Therapist to ensure the quality delivery of all ordered care. Review respiratory assessments and intervenes as needed. Monitor the quality and quantity of care provided by non-licensed staff. Review and approve documentation of care provided by non-licensed staff. Participate in ISP meetings as requested. Provide instruction, guidance, and assistance to other members of the health care team as well as resident's families. Communicate with leadership team program's needs, strengths, weaknesses, and overall progress
OTHER DUTIES :
- Participate in the services trainings and meetings as needed.
- Attain and maintains trainer certification in Safety Care/First Aid and CPR.
- Performs such other duties as assigned by supervisor.
QUALIFICATIONS :
- Strong oral and written skills, and the ability to communicate well with others.
- Knowledge of the observation and treatment of illness and disease.
- Knowledge of medications and their prescribed usage; technical knowledge of medication administration principles.
- Ability to work with families.
- Good organizational, decision-making, and customer service skills.
- Reliability, adaptability, and flexibility, including willingness and ability to maintain a good attendance record.
- Ability to follow written program plans and document, and maintain records related to them.
- Knowledge of, or ability to obtain, computer skills sufficient to complete assigned job duties.
- Ability to work with others as part of a team in order to satisfy the needs of individuals, the program and the Agency.
- Must demonstrate knowledge of, and willingness to adhere to, all departmental and Agency policies and procedures.
- Physical condition commensurate with demands of the job, including the unrestricted ability to assist in lifting and transferring on a regular basis.
EXPERIENCE/EDUCATION REQUIRED :
- Currently licensed in the State of Virginia to practice as an LPN.
- High School Diploma/GED required.
- One to three years' nursing experience including knowledge and experience in the field of developmental disabilities, serious mental illness, long-term care, or community health experience.
- Valid Driver's license with a safe driving record.
