DUTIES AND RESPONSIBILITIES:
- Manages a small caseload of client families, providing services of an intensive nature and utilizing short-term, solution focused treatment strategies.
- Prepares written diagnostic assessments, assessing client from multiple perspectives, including family functioning, psychological/emotional, social, academic, medical, behavior, substance abuse, skills, strengths, barriers to economic self-sufficiency, treatment history, and others, and utilizing various assessment instruments
- Facilitates wraparound team meetings to enlist existing supports in determining and meeting service objectives and to identify additional resources and supports
- Prepares written service plan, incorporating input from clients, involved service providers, and wraparound team members, and clearly identifying problem areas and needs, strategies, and service objectives
- Provides direct services to clients, including individual, family, couple, and group sessions, and issue specific or educational groups (parenting skills, anger management, substance abuse interventions)
- Responds to crisis intervention with twenty-four (24) hour a day availability
- Documents case activities for both clinical and billing purposes
- Prepare progress reports as required by referral source or service purchaser
- Participates in Child Specific Team, Family Assessment and Planning Team, and other interdisciplinary, diagnostic, or planning meetings
- Collaborates and coordinates with other case involved service providers and professionals, such as attorneys, school personnel, social workers, and probation workers
- Links clients with external programs or services, such as health services, recreational activities, child care services, financial resources, employment resources, child care, AA/NA groups
- Attends meetings, trainings and conferences as required.
- Attends bi-weekly supervision with the Program Director.
- Performs other duties as assigned by the Executive Director and Program Director of Seven Summits Family Services.
EDUCATION AND EXPERIENCE
The Community Based Solutionist shall have at minimum a Bachelor’s Degree in social work, sociology, psychology, education, counseling or other behavioral science field and one year of experience providing services to at-risk youth and their families. The In-home worker shall also meet the qualifications for QMHP-C registration in the state of Virginia and be either a registered Qualified Mental Health Professional or Qualified Mental Health Professional-Trainee.
KNOWLEDGE, SKILLS AND ABILITIES
Knowledge of community based services, commonly used concepts, practices, ethics and procedures within the clinical human services field. Knowledge of the Department of Behavioral Health and
Developmental Service’s regulations, DSM V diagnoses, and psycho-social assessments. Knowledge of the referral process, therapeutic groups, community resources, Multisystem Theory and therapeutic crisis intervention training. Knowledge of computers and how to use applications with Microsoft Office.
Skills in oral and written communication, dealing with the public, remaining calm under pressure, problem solving with clients, and time management. Skilled in attending to details, managing work activities, completing multiple tasks simultaneously and meeting deadlines. Skilled in understanding and following directions.
Ability to establish and maintain professional work relationships with fellow workers, problem solve, and implement procedures and standards of the Seven Summits Family Services program. Ability to assist Workers and workers on the program orientation and requirements, provide instructions and organize tasks and complete them within required time frames. Ability to pay attention to detail, identify problems and present possible solutions to the Program Director. Ability to manage multiple projects simultaneously and report on progress. Ability to be assertive and make sound clinical judgments, and present the program and market to referral sources.
