Key Responsibilities

• Coordinate care for assigned clients and develop goal-oriented care plans, including Start of Care and ongoing reassessments

• Conduct onsite supervisory visits to evaluate clients, family environments, and caregivers, and complete timely documentation

• Ensure availability, proper use, and maintenance of required equipment and supplies

• Provide supervision, training, orientation, and ongoing education to clinical field staff

• Deliver excellent customer service and maintain strong communication with clients and families

• Provide nursing updates and obtain re-authorization for continued care

• Develop ISP plans and quarterly reports for DD waiver clients using the WAMS system

• Ensure compliance with DMAS regulations and agency policies (preferred knowledge)

• Maintain professional nursing standards in all interactions and documentation

• Perform additional duties as assigned, including caregiver and family training when required

Qualifications

• Active Registered Nurse (RN) license required

• CPR and First Aid certification required

• COVID-19 vaccination required

• Minimum of 2 years of home care experience preferred

• Willingness to travel within Fairfax, VA and surrounding counties

• Strong understanding of home health care operations and compliance requirements

Preferred Qualifications

• Experience with DD Waiver programs

• Home health care experience

• CCC+ Medicaid experience

• Working knowledge of DMAS regulations and compliance standards
