Responsibilities
- Provide direct patient care to residents in a long-term care setting, following individualized care plans and established policies
- Administer medications and monitor for adverse effects in collaboration with the supervising nurse and physician orders
- Document care activities accurately in the resident’s chart and communicate changes to the RN or physician promptly
- Monitor vital signs, assess residents for safety and comfort, assist with activities of daily living, and implement fall-prevention and mobility interventions
- Maintain infection control standards, support wound care and device care as prescribed, and ensure proper environmental hygiene
- Collaborate with the interdisciplinary team to coordinate care, report changes, and support resident well-being and family communication

Requirements
- Active LPN license in Virginia (or eligible for licensure by endorsement)
- Prior long-term care or skilled nursing experience preferred
- Ability to work 12-hour day shifts (07:00–19:00) and participate in a travel assignment
- Strong documentation and communication skills; ability to follow care plans and relay changes to the team
- Ability to pass background check and drug screen
- Proficiency with electronic health record systems or basic computer skills
