Roles & Responsibilities

• Evaluate residents' physical therapy needs and develop personalized treatment plans
• Implement therapeutic exercises and interventions to improve mobility, strength, and balance
• Collaborate with physicians, nurses, and rehab team members to deliver coordinated care
• Document patient progress and modify plans to achieve optimal outcomes
• Educate patients and families on safe movement, fall prevention, and home exercise programs
• Provide compassionate, patient-centered care that supports residents' quality of life

Qualifications

• Active Physical Therapist (PT) license in Virginia or eligibility to obtain one
• Doctor of Physical Therapy (DPT) degree
• New graduates are welcome - any experience level accepted
• Previous SNF or geriatric rehab experience is a plus but not required
• Strong communication skills and a collaborative spirit
• Reliable, compassionate, and committed to patient care

Education

• Doctor of Physical Therapy (DPT)

Knowledge & Skills

• Knowledge of physical therapy assessment and treatment techniques
• Understanding of skilled nursing facility regulations and documentation practices
• Skill in developing and modifying therapeutic exercise programs
• Ability to work effectively with older adults and interdisciplinary teams
• Strong clinical reasoning, problem-solving, and organizational skills
• Familiarity with electronic health records and therapy documentation
