Position Summary:

FACETS is seeking mission-driven Case Managers to join our team providing comprehensive, community-based case management services to individuals experiencing homelessness or housing instability. Case Managers help clients access essential services, secure stable housing, and work toward long-term stability and self-sufficiency.

Key Responsibilities

• Conduct comprehensive assessments to identify client needs and develop individualized service plans
• Connect clients to housing resources, healthcare services, and community-based supports
• Assist with housing applications, documentation, and housing stability planning
• Conduct field-based outreach to engage individuals experiencing homelessness, including visiting shelters, encampments, and other community locations
• Coordinate referrals to medical, behavioral health, and social service providers as appropriate
• Maintain accurate and timely case documentation in required systems
• Collaborate with internal teams and community partners to support client goals

Required Skills and Abilities

• Bachelor’s degree in social work, human services, or a related field, plus two years of professional social work or case management experience working with homeless and/or at-risk populations
• Ability and willingness to conduct field outreach, including visiting wooded areas or locations not intended for habitation to engage program participants
• Strong written and verbal communication skills
• Proficiency with Microsoft Office Suite, including Microsoft Excel
• Valid driver’s license, reliable transportation, good driving record, and personal auto insurance required
• Ability to drive a 15-passenger van
• Candidates must successfully pass required criminal background checks
• Bilingual (English/Spanish) is preferred.

Physical Requirements:

• Prolonged periods of sitting at a desk and working on a computer.
• Must be able to lift 15 pounds at times.
• Must be able to attend occasional meetings at other FACETS office locations within Fairfax County, including 3 annual organization-wide fundraising events each year.

Salary Range:

$48,000 - $51,000 annually
