Position Overview:

We are seeking an experienced Hospice RN Care Manager to provide compassionate, patient-centered end-of-life care throughout the Roanoke Valley region. This field-based role offers the opportunity to work closely with patients and families while delivering expert symptom management, care coordination, and clinical support.

As the primary clinical point of contact, you will collaborate with an interdisciplinary care team to develop and implement individualized care plans that promote comfort, dignity, and quality of life.

Responsibilities:

Patient Care & Case Management:

• Manage a caseload of hospice patients within the community.
• Conduct comprehensive nursing assessments and home visits.
• Monitor and manage symptoms, pain, and clinical changes.
• Provide crisis intervention and symptom management when needed.
• Develop and adjust individualized care plans.
  

Care Coordination:

• Collaborate with physicians, social workers, chaplains, therapists, and caregivers.
• Coordinate services to ensure seamless patient care.
• Maintain accurate and timely clinical documentation.

Family Education & Support:

• Educate patients and families on medications, disease progression, and comfort measures.
• Provide emotional support and guidance throughout the hospice journey.
• Build trusted relationships with patients and caregivers.
  

Qualifications:

Required:

• Active Virginia RN License or Compact RN License.
• Minimum one (1) year of hospice or palliative care nursing experience.
• Valid driver's license.
• Reliable transportation.
• Strong assessment, communication, and clinical decision-making skills.
  

Preferred:

• Previous hospice case management experience.
• Experience working independently in community-based settings.
