RN Case Management Job Details

• Location: Warrenton, VA 20186 (Virginia healthcare jobs)
• Facility: Acute Care Hospital (97 beds)
• Position Type: Locum tenens, Travel RN
• Specialty: Case Management / Discharge Planning
• Shift: 8-hour day shifts (08:30–16:30), rotating weekends
• Hours: 40 hours/week (Guaranteed)
• Assignment Length: 13 weeks (Start: 9/15/2025 – End: 12/15/2025)
• Patient Population: Adults, geriatrics, OB/newborn
• Electronic Health Record: Meditech; Careport for discharge planning
• Dress Code: Navy blue scrubs or business casual
• Estimated Weekly Salary Range: $2000-$2050

Registered Nurse Qualifications

• Active RN license in Virginia or compact state (Virginia nursing jobs)
• BLS certification required
• Minimum 1 year of recent case management or discharge planning experience (5 years preferred)
• Strong organizational and clinical support skills
• Outgoing, friendly, and quick learner
• Ability to float and provide coverage for all hospital units

RN Case Management Responsibilities

• Coordinate patient care and discharge planning for diverse patient populations
• Utilize Careport and Meditech for documentation and care coordination
• Collaborate with interdisciplinary healthcare teams to ensure safe and effective transitions of care
• Maintain patient ratios of 15–20:1 while delivering high-quality case management services
• Participate in weekend rotation as scheduled
• Demonstrate professionalism and adaptability in a fast-paced clinical environment
