Core Responsibilities:

• Conduct thorough adult and geriatric health assessments, including medical history, diagnosis, treatment, and follow-up care.
• Administer and order necessary lab tests and point-of-care testing.
• Collaborate effectively with physicians, case managers, social workers, family members, and other medical staff.
• Provide health education and support to ensure follow-up and comprehensive care for patients in their homes or skilled nursing facilities.

Qualifications:

• Master's Degree or equivalent experience in nursing or healthcare.
• Current NP licensure with prescription authority in your state.
• Relevant board certification (AANP, AACN, or ANCC) in your specialty.
• BLS, ACLS, or CPR certification.
• Preferred one year of NP experience, though new graduates are encouraged to apply.
• Valid driver's license and automobile insurance that meets state requirements.
• Strong computer skills and effective communication abilities.
