Schedule: Monday - Friday, every 8th weekend

We’re looking for an RN Case Manager who is as committed to quality care as they are to human connection. This role is more than clinical oversight—it’s about guiding patients and families through their healthcare journey with dignity, clarity, and kindness.

How You'll Make a Difference

• Serve as the primary nurse for your assigned patients, developing and coordinating individualized care plans.
• Conduct in-home assessments, provide skilled nursing care, and adjust plans based on evolving patient needs.
• Collaborate with a caring interdisciplinary team to ensure seamless communication and continuity of care.
• Support families with education and emotional guidance through every step of the care journey.
• Maintain thorough documentation in compliance with state, federal, and organizational standards.
• Participate in case conferences and team meetings—your voice and input matter.

Requirements

• Active RN license in good standing
• Strong assessment and critical thinking skills
• Self-motivated and comfortable working independently
