What You'll Do:
Conduct comprehensive assessments of patients with chronic illnesses
Develop and implement individualized care plans
Collaborate with multidisciplinary teams to ensure optimal patient outcomes
Monitor patient progress and adjust treatment plans as necessary
Educate patients and families on chronic disease management strategies
️ Advocate for patients' needs within the healthcare system
What We're Looking For:
✅ A valid RN license with at least 1 year of experience in chronic care nursing
✅ Strong critical thinking and problem-solving skills
✅ Excellent interpersonal and communication skills
✅ Ability to work effectively in a fast-paced environment
