Overview
Registered Nurse – Hospice (RN Care Manager) travel assignment in Roanoke, Virginia. Start date 07/13/2026 for 13 weeks. Weekly pay range $1,963–$2,008. Guaranteed hours: 0. Shift: 5x8 Days (8:30 AM–5:00 PM). Work setting: Home healthcare – Hospice. This assignment focuses on providing compassionate, patient-centered hospice care in the patient’s residence with travel to long-term care facilities, hospitals, and area agencies as needed.

Responsibilities
- Provide expert symptom management, crisis intervention, and comfort-focused care to hospice patients in their homes.
- Develop, implement, and update individualized care plans in collaboration with an interdisciplinary team; educate patients and families on disease processes, treatments, and end-of-life care.
- Monitor patient status, assess changes, administer medications as prescribed, and intervene promptly to prevent complications.
- Document care activities accurately in the patient record and communicate changes to the care team; participate in care conferences and care coordination activities.
- Travel to patient residences and to affiliated facilities or agencies as needed within the service area; support families through transitions and bereavement education when appropriate.
- Maintain professional standards, ensure patient safety, and comply with applicable regulations and agency policies.

Requirements
- Active Virginia RN license
- 1+ year hospice experience required
- Valid driver’s license with a good driving record and reliable transportation
- Ability to travel within the hospice service area; overnight travel may be necessary for continuing education
- Comfort working in patients’ homes and in diverse settings; strong communication and team collaboration skills
