Job Description

Description:

Psychiatric Mental Health Nurse Practitioner (PMHNP) – Virtual Care

At CloseKnit, our mission is to make care more convenient, connected, and human—so it fits into real life, not the other way around. The Psychiatric Mental Health Nurse Practitioner (PMHNP) – Virtual Care plays a vital role in advancing this mission by delivering compassionate, evidence-based psychiatric care through our secure, virtual-first platform.

This role provides comprehensive mental health services—including evaluations, diagnosis, medication management, and ongoing treatment planning—while collaborating closely with an interdisciplinary care team to ensure seamless, whole-person care. As part of CloseKnit’s “Your Healthcare Anywhere” model, the PMHNP helps expand access to high-quality behavioral health services, empowering patients to live healthier, fuller lives through thoughtful, patient-centered virtual care.

Position Summary

The Psychiatric Mental Health Nurse Practitioner (PMHNP) provides comprehensive, evidence based psychiatric care to patients through a secure, fully virtual care platform. This role is responsible for conducting psychiatric evaluations, diagnosing mental health conditions, managing psychiatric medications, and collaborating with an interdisciplinary behavioral health team to deliver high quality, patient centered treatment.

This position is designed as part-time preferred. If you're looking for a position that allows a flexible schedule, this position might be right for you! Clinical services will be delivered entirely via telehealth initially but could transition into a hybrid model depending on organizational needs.

Key Responsibilities
  • Conduct comprehensive psychiatric evaluations using evidence-based assessment tools.
  • Diagnose and treat a wide range of mental health conditions, including but not limited to anxiety disorders, depressive disorders, eating disorders, OCD, substance use disorders, ADHD, and personality disorders.
  • Prescribe, manage, and monitor psychiatric medications in accordance with clinical guidelines and state regulations, assessing treatment effectiveness, side effects, and patient adherence.
  • Develop, implement, and adjust individualized treatment plans in collaboration with the interdisciplinary clinical team.
  • Coordinate care with therapists and other healthcare providers, including referrals to additional services when clinically appropriate.
  • Provide patient education related to diagnoses, treatment options, and prescribed medications.
  • Maintain timely, accurate, and thorough clinical documentation of patient encounters, treatment plans, and progress notes.
  • Ensure compliance with all applicable legal, ethical, and organizational standards.
  • Participate in quality improvement initiatives and support continuous clinical excellence within a virtual care environment.
Equal Opportunity Statement

  • Commitment to delivering compassionate, culturally responsive, and patient centered care.
