Description

American Traveler is hiring an RN Case Manager for a Hospice/Palliative Care position in Loudoun County, VA, requiring a minimum of 1 year of recent hospice experience and a valid VA or compact RN license.

Details

• Community/home-based hospice setting operating out of a hub office in Loudoun County, VA
• Geriatric patient population requiring end-of-life care
• Caseload of approximately 5–7 patients per day
• Day shift schedule, 5x8-hour shifts (8:00 AM – 5:00 PM)
• No on-call responsibilities
• Territory covers up to a 60-mile radius from the hub office; flexibility with territory is expected
• EMR: Netsmart
• iPad provided for use in the field

Requirements

• Active VA RN license or compact RN license currently in hand
• Current BLS certification required
• Minimum 1 year of recent hospice nursing experience required
• Strong time management skills with an emphasis on timely documentation and communication

Additional Information

• Responsibilities include managing a caseload of hospice patients in a community/home-based setting across an assigned territory
• Scrubs required in any color; jeans are not permitted
• Mileage is reimbursed at the IRS standard rate
