RN CASE MANAGER (HOSPICE) | DETAILS AND COMPENSATION:

• Location: Colonial Heights VA 23834 – 100% On-site/Fieldwork
• • and surrounding areas (Chesterfield, Tri-Cities, Hopewell, Petersburg)
• Payrate: $56.14/hr
• Required Availability: Full-Time | Monday–Friday, 8:00 AM–4:30 PM
• • (some on-call required as needed)

RN CASE MANAGER (HOSPICE) | SUMMARY AND HIGHLIGHTS:

The RN Case Manager role focuses on providing holistic, patient-centered care coordination and ensuring continuity across settings while supporting patients and families through their healthcare journey.

• This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs
• Benefits are available to full-time employees after 90 days of employment and include health, optical, dental, life, and short-term disability insurance
• A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates

RN CASE MANAGER (HOSPICE) | RESPONSIBILITIES:

• Perform comprehensive assessments of high-risk members and develop individualized care plans.
• Collaborate with physicians, social workers, and care teams to ensure continuity and quality of care.
• Address both medical and non-medical needs, including arranging transportation, housing, or other support services.
• Coordinate care transitions between facilities and community settings.
• Monitor and evaluate clinical, functional, and fiscal outcomes, maintaining accurate and detailed documentation.
• Identify opportunities for health promotion, illness prevention, and cost-effective care.
• Serve as a patient advocate, ensuring confidentiality and adherence to care standards.

RN CASE MANAGER (HOSPICE) | REQUIREMENTS :

• Education: Associate’s or Bachelor’s degree in Nursing or a related field.
• Attendance: Commitment to mandatory attendance during the first 90 days.
• Active Registered Nurse (RN) license in Virginia or Compact State .
• Hospice experience required
• HCHB (HomeCare HomeBase) experience preferred.
• Strong ability to interpret medical information and make informed decisions.
• Case Management certification preferred.
