- Contract Duration: 13 weeks
- Nursing Shift: Day shift, Monday–Friday, 8:00 AM – 4:30 PM
- Hours per Week: 40
- Estimated Weekly Salary: $1500–$1540
- Charting System: Epic (Epic EMR experience required)
- Active Licensed Practical Nurse (LPN) license in Virginia or compact state (must be in hand at time of submission)
- Minimum 2 years of recent home health nursing experience
- BLS Skills certification required
- Proficiency with Epic electronic medical records (EMR)
- One supervisory reference required for submission (two for compliance)
- Specialty homecare experience within the last year
- Ability to float to other home health units as needed
- Deliver compassionate home health care to adult patients in the Accomac community
- Monitor and document vital signs, blood glucose, intake/output, and pain levels
- Communicate changes in patient status to Registered Nurse (RN) or Medical Doctor (MD)
- Conduct patient health questionnaires, medical histories, and allergy assessments
- Assist with end-of-life care and implement isolation precautions as needed
- Coordinate pharmacy calls and manage patient medication refill requests
- Accurately document patient care using Epic EMR
- Participate in interdisciplinary healthcare team meetings and support patient-centered care
