Director of Care Coordination

GENERAL PURPOSE

Under the supervision of the Vice President of Care Coordination, the Director of Care Coordination is responsible for the supervising the delivery of services provided by the Care Coordinators, coordinating the care of clients aged 60+, being the lead for the GUIDE Model/Caregiver Programs, promoting equity through the identification and removal of barriers, facilitation of learning, cultivation of community, and empowerment of people.

POSITION RESPONSIBILITIES

· Conduct person centered assessments of clients aged 60+, and their caregivers, to determine needs and goals and develop a plan of care.

· Coordinate agency and community services by providing information, making referrals, assisting with completing applications, and facilitating service delivery to ensure the client care plan is successful.

· Maintain complete client files and electronic records in accordance with PAA policies and procedures.

· Provide (LCSW) or arrange counseling services for clients and family caregivers.

· Research and advocate for appropriate public assistance resources for clients.

· Assist Human Resources to design and lead professional development for PAA staff on topics that improve individual knowledge, skills, and abilities related to service provision.

· Assist with grant writing as required.

· Participate in ongoing training as needed in order to meet all certification standards and credentialing policies.

· Refer all suspected situations of adult abuse, neglect, and exploitation to the Department of Social Services.

· Attend staff meetings and other agency activities as required.

· Perform other tasks related to the position.

ESSENTIAL SKILLS AND EXPERIENCE

· Education: Master’s Degree from an accredited institution in Social Work or related field. LCSW or LPC required.

· Experience assessing, coordinating services for, and interviewing older adults and caregivers.

· Comprehensive working knowledge of new and ongoing programs and services available to assist persons 60+ and their caregivers.

· Knowledge of current gerontological best practices, person centered practices, trauma informed care, and crisis intervention techniques.

· Excellent oral and written communication skills.

· Ability to establish and maintain effective working relationships and communicate effectively with clients, other professional staff, community agencies, and the general public.

· Ability to prepare reports, maintain accurate records, and accurately enter client assessment data into electronic records.

· Commitment to promoting equity through the identification and removal of barriers, facilitation of learning, cultivation of community, and empowerment of people.

· Valid driver’s license and use of personal vehicle.

Specific vision abilities required by the job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus.

· Work environment : While performing the duties of this job, the employee is exposed to weather conditions prevalent at the time. Client interviews are mostly conducted in private homes and employees may be exposed to smoke, pets and unsanitary conditions. The noise level in the work environment is usually minimal.
